PURPOSE & MISSION STATEMENT
Purpose:
Iowa Nursing and Rehab exists to provide Home and Community-Based Services that support
members in accessing the benefits of community living in settings that are integrated in, and
support full access to, the greater community. Services are designed to ensure members
receive support in a manner that promotes individual choice, autonomy, independence,
dignity, and respect.
MISSION:
The mission of Iowa Nursing and Rehab is to deliver Home and Community-Based Services
that are person-centered, responsive to individual needs, and consistent with federal and
Iowa HCBS Settings requirements. Services support members in exercising choice and
control over their daily lives, engaging in meaningful activities, and living in settings that reflect
their personal preferences, goals, and needs.
HCBS Service Principles
Person-Centered Planning and Delivery
Services and supports are developed through a person-centered planning process and are
based on the individual’s strengths, preferences, needs, and desired outcomes, as identified
in the Person-Centered Service Plan (PCSP).
Member Rights, Dignity, and Choice
Members are afforded the same rights as individuals not receiving HCBS, including the right
to privacy, dignity, respect, and freedom from coercion and restraint. Members are supported
to make informed choices regarding services, daily routines, and life activities.
Independence and Autonomy
Services support members in exercising autonomy in making decisions about daily schedules,
activities, personal goals, and access to food, visitors, and community resources, consistent
with health and safety needs.
Health, Safety, and Well-Being
Services are delivered in a manner that ensures member health and safety while maximizing
independence and personal choice. Any limitations on rights are individualized, justified,
documented in the PCSP, and reviewed as required.
Community Integration and Inclusion
Members are supported to engage in community life, seek employment and volunteer
opportunities when desired, and access community services and activities in the same
manner as individuals not receiving HCBS.
