Program Overview

Iowa Nursing and Rehab provides Host Home and Group Home services for individuals with disabilities who require supportive, community-based residential environments. These services are designed to promote independence, dignity, personal choice, and meaningful community inclusion while ensuring the health, safety, and well-being of each member.


Host Home Services

Host Home services provide individuals with the opportunity to reside in a private home setting with a qualified Host Home Provider. Supports are delivered in a family-style environment and are individualized based on the member’s needs and preferences. Services focus on maintaining a safe and stable living environment while supporting daily living skills, independence, relationship development, and participation in community activities.


Group Home Services

Group Home services provide structured residential supports in shared, community-based homes for individuals who require a higher level of daily assistance. These services are designed to support health and safety, skill development, social interaction, and access to community resources while promoting independence, choice, and self-determination.


Person-Centered Approach

All Host Home and Group Home services are developed and delivered through a person-centered planning process. Supports are based on the individual’s strengths, preferences, needs, and desired outcomes as documented in the Person-Centered Service Plan (PCSP). Members are supported to exercise choice and control over daily routines, activities, and life decisions to the greatest extent possible.


Community Inclusion and Daily Life

Community inclusion is a fundamental component of Host Home and Group Home services. Members are encouraged and supported to participate in community activities, develop natural supports, and access community services and resources in the same manner as individuals not receiving Home and Community-Based Services (HCBS).


Health, Safety, and Well-Being

Iowa Nursing and Rehab prioritizes the health and safety of all members while promoting independence and autonomy. Services include trained and qualified providers, proactive wellness monitoring, emergency preparedness planning, and respectful, trauma-informed support practices delivered in accordance with applicable HCBS standards and regulations.

Personal Care Services
